What the packet usually contains
A registration form with your contact and insurance details, a medical history questionnaire, a form confirming you received the practice's privacy notice, a financial or payment policy, and sometimes consent to treatment and communication preferences (for example, whether they may leave voicemails or text you).
The privacy notice acknowledgment
Many practices ask you to sign that you received their notice of privacy practices. Signing it confirms you received the notice; it doesn't give up your rights. Read the notice if you want to know how your information is used.
Sending the forms safely
Use the practice's patient portal or the method they recommend. Forms with health information are sensitive — avoid sending them to unverified addresses. Here, the PDF is filled and signed on your device and isn't uploaded to us.
